Form 940 Worksheet Columns
The Form 940 Worksheet contains a standard set of columns. You cannot select columns for this report. The header area of the Form 940 Worksheet displays the Form W-2 year and quarter for which you generate the report. The body of the worksheet displays gross pay, 125/Cafeteria contributions, and total wages subject to tax for each employee.
Columns
| Column | Description |

| Employee Number/ Name | This column displays the employee's identification number and name. | 
| Soc. Sec. No. | This column displays the employee's Social Security number. | 
| Gross Pay | These columns display the employee's gross pay for the reporting periods. | 
| 125/Cafeteria | These columns display the 125/Cafeteria contributions withheld for the employee for the reporting periods. | 
| Add'l Exempt Payments | If you excluded types of withholding or employer contributions from taxable wages using Additional Exempt Payments on the General tab, these columns display the total excluded amount for the reporting periods. | 
| Subject to Tax | These columns display the total amount subject to tax for the reporting periods, calculated as follows: Gross pay - 125/Cafeteria contributions. The maximum amount subject to tax is the annual wage base that you specified on the General tab. |
