Purchase Requisition Form Columns

The Purchase Requisition Form contains a standard set of columns. You do not have the option to select columns for this report.

Columns

Column Description
Comments If comments were entered on the Comments tab of the Purchase Requisitions form, those comments display in the upper left part of the form.
Requisition Number This column displays the purchase requisition number.
Requisition Date This column displays the date that the purchase requisition was created.
Requestor Name This column displays the name of the employee who requested the items.
Requestor Signature This column displays a space in which the person who requested the items can sign the requisition.
Authorized By This column displays the name of the employee who authorized the purchase requisition.
Requisition Status This column displays the status of the purchase requisition (for example, Approved, Rejected, In Progress, Submitted, or Closed).
Ship To This part of the form displays the full address to which you want the items shipped.

The ship-to address can include additional contact information, such as a phone number, fax number, or email address.

If you select the Modify Shipping Contact and Email option on the General tab of the Purchasing & Inventory Company Settings form, the Ship To and E-mail fields in both the header and detail lines display modified data when appropriate.

Item This column displays the identifier for the item or service.

You create and maintain purchasing item records on the Items Master form (Settings > Purchasing & Inventory > Items Master).

Description This column displays the description of the item or service.
Qty This column displays the number of items requested.
U/M This column displays the unit of measure for the items (for example, Each, Box, Case, Dozen, or Hour).
Unit Price This column displays the price per unit for the items. The unit price always has a currency symbol, whether or not the default currency format on the Layout tab includes the currency symbol.
Total This column displays the total cost for the requested quantity of the item. The total cost always has a currency symbol, whether or not the default currency format on the Layout tab includes the currency symbol.
Due Date This column displays the due date for the requested item.
Project If the cost of the item is distributed to projects, phases, or tasks, this column displays the project number.
Name This column displays the project name.
Phase If the cost of the item is distributed at the phase level, this column displays the phase number.
Task If the cost of the item is distributed at the task level, this column displays the task number.
Billable This Yes/No column indicates whether or not the cost of the item is billable to the project, phase, or task. If multiple projects, phases, or tasks share the cost of a requested item, some of the cost can be billable while the rest is not.
Percentage/Unit Multiple projects, phases, or tasks can share the cost of a requested item. In that case, DPS distributes the cost to projects based on either percentages or units. Which is used depends on the setting on the Cost Distribution tab of the Purchase Requisition Line Detail dialog box.
  • If the distribution is by percentage, this column displays the percentage of the overall cost distributed to each project, phase, or task. For example, DPS could distribute 80% of the cost to a project and the other 20% to a phase in a second project.
  • If the distribution is by units, this column displays the unit cost distributed to the project, phase, or task.
Vendor Number This column displays the number for a suggested vendor for the requested items or services.
Vendor Name This column displays the name of a suggested vendor for the requested items or services.
Vendor Item This column displays the item number used by the vendor.
Price Quote? This Yes/No column indicates whether or not the suggested vendor has provided a price quote.
