Use the drop-down box to select the order of importance for the emergency contact information. The options are:

• Primary - Select "Primary" to indicate that this contact is first in the order of importance of your emergency contacts.  "Primary" is the system default.

• Secondary - Select "Secondary" to indicate that this contact is second in the order of importance of your emergency contacts.

Enter up to a 25-character, alphanumeric contact name.

Note: This field is required if the Relationship or Phone field displays on the screen.

Enter up to 15 alphanumeric characters to describe the relationship of this contact to you.

Note: This field is required if the Name or Phone field displays on the screen.

Enter up to 10 alphanumeric characters for the phone number.

Note: This field is required if the Name or Relationship field displays on the screen.

This non-editable field shows how the phone number should be entered on screen.

Enter up to 30-character alphanumeric characters for the street address, line 1.

Enter up to 30-character alphanumeric characters for the street address, line 2.

Enter up to 30-character alphanumeric characters for the street address, line 3.

Enter up to 25 alphanumeric characters for the contact's address city.

Use the drop-down box to select the state/province for the address entered/displayed on the screen.  The valid values are the states/provinces set up in the system for the selected country.

Enter up to 10 alphanumeric characters for the postal code.

This non-editable field shows how the postal code should be entered on screen.

Use the drop-down box to select the country for the address entered/displayed on screen.  The valid values are the countries set up in the system.
